Normal AG Metabolic Acidosis
GI loss of HCO3
  • Diarrhea
  • Enterocutaneous fistula (e.g., pancreatic)
  • Enteric diversion of urine (e.g., ileal loop bladder)
  • Pancreas transplant with bladder drainage
Renal loss of HCO3
  • Proximal renal tubular acidosis (RTA) (type 2)
  • Carbonic anhydrase inhibitors
Failure of renal H+ secretion
  • Distal RTA (type 1)
  • Aldosterone deficiency/resistance RTA (type 4)
  • Early renal failure (GFR > 20 ml/min)
